Prepare for Your Visit

Bring the right documents and medical information to help your DOT physical move as smoothly as possible.

Bring these items

  • Driver's license or other photo ID
  • A current medication list
  • Glasses, contacts, or hearing aids if you use them
  • Your current medical card, if you are renewing

If these apply to you

  • CPAP compliance report for sleep apnea treatment
  • Blood sugar records, A1C results, or diabetes-related documentation
  • Cardiology notes, stress test results, or specialist clearance letters
  • Skill Performance Evaluation (SPE) certificate, if applicable
  • Any paperwork you have been told to complete ahead of the visit

Forms and paperwork

Scheduling is handled through an external booking portal. If intake forms are assigned during booking, complete them there before your appointment.

If you need additional DOT-related paperwork because of a medical condition, bring the records you already have and any specialist letters available.

Helpful tip

Missing documentation is one of the most common reasons a DOT certification cannot be completed the same day. If you are unsure what records to bring, call before your visit.
